What end-of-life care includes

End of life care focuses on comfort, dignity, and practical support for individuals and families.

  • Personal care, including bathing, dressing, grooming, and repositioning

  • Assistance with mobility, transfers, and daily routines

  • Meal preparation, hydration support, and assistance with eating

  • Medication reminders following existing care plans

  • Overnight, extended-hour, or around-the-clock support

  • A calm, reassuring presence and companionship

  • Monitoring and communicating changes in condition

  • Coordination with family members and healthcare providers

  • Support for family caregivers during periods of increased need

  • Maintaining familiar routines and surroundings whenever possible

Care plans are tailored to individual needs and can work alongside publicly funded services, nursing care, physicians, hospice teams, and other healthcare providers.
