What end-of-life care includes
End of life care focuses on comfort, dignity, and practical support for individuals and families.
Personal care, including bathing, dressing, grooming, and repositioning
Assistance with mobility, transfers, and daily routines
Meal preparation, hydration support, and assistance with eating
Medication reminders following existing care plans
Overnight, extended-hour, or around-the-clock support
A calm, reassuring presence and companionship
Monitoring and communicating changes in condition
Coordination with family members and healthcare providers
Support for family caregivers during periods of increased need
Maintaining familiar routines and surroundings whenever possible
